This Privacy Policy explains how Continence Assessment Australia collects, uses, stores and shares personal and health information when you enquire about, book or receive a continence assessment.
We are committed to handling your information with care and in line with the Australian Privacy Principles under the Privacy Act 1988 (Cth). Clinical delivery is provided in partnership with Caremmunity, and your information may be shared between the two organisations to the extent needed to provide our services.
The information we collect
We only collect information that is reasonably necessary to provide our services. This may include your name, contact details, date of birth, location, and information about your health, symptoms, daily routines, support needs and continence products.
We may also collect information about how your assessment will be funded, such as your NDIS or Support at Home arrangements, and the details of any support coordinator, plan manager, GP or authorised representative involved in your care.
How we use your information
We use your information to assess your continence needs, arrange your appointment, prepare your continence management plan and report, and communicate with the people you authorise us to contact. We may also use de-identified information to review and improve the quality of our service.
We do not sell your personal or health information, and we do not use it for marketing purposes without your consent.
How we share your information
With your consent, we may share your report and relevant information with your GP, support coordinator, plan manager, or another health provider or organisation you nominate.
We may also share information where the law requires or allows it, such as to meet our legal or regulatory obligations or where there is a serious and imminent risk to a person's health or safety.
Clinical records are shared with our clinical delivery partner, Caremmunity, in a manner consistent with your consent. Any third-party service providers we use to store or transmit records are required to keep that information confidential and secure.
How we protect your information
We use secure systems to store records and send documents, and we take reasonable steps to protect your information from misuse, interference, loss, and unauthorised access, modification or disclosure.
Wherever we share information electronically, we use secure channels. No system is completely risk-free, but we take proportionate steps to safeguard the information we hold about you.
Accessing and correcting your records
You have the right to ask for access to the personal information we hold about you, and to ask us to correct it if it is inaccurate, incomplete or out of date. You can make a request by contacting us using the details below.
In some limited circumstances the law may permit us to refuse access to certain information. If we do, we will explain our reasons where we are able to do so.
